Add Pages to your Questionnaire
You can add pages in between questions forcing each question to be on a screen by itself in Design View. By default, questions will be created on the same page (screen) forcing the respondent to scroll vertically if there are too many questions on a page. To make it more user-friendly for the respondent and avoid scrolling, you may want to limit your screens to just one or two questions per screen. Typically, the designer puts one question on a screen at a time. Screens with multiple questions cannot have logic based on a question on the same screen. The answer to your question needs to be submitted before you can base logic off of it.
When adding a new page, you have the ability to add text to the top of the page (optional). This text will appear at the top of the page along with the next question.

Click the Design View tab

On the left side of the screen, click the New Page drop down menu

Select where you want to add a new page in the questionnaire

Click the “Go” button

On the right side of the screen, your page will be added and the Question Editor will automatically execute
In this example we are adding a new page after the question Q1 and before the question MALEGOTO.
A page has been inserted between the question Q1 and the MALEGOTO question. We are also adding text to the top of this page that will appear on the same screen as the MALEGOTO question. This text is optional. After typing the text, we are clicking the “submit” button to add the page.
